Download and complete these forms after your first meeting with Dr. Kaye. These forms are best done in 2-3 sittings. If you are a couple, you will need a set of forms for each person. If you are meeting with Dr. Kaye in person, you can bring your hard copies to your next in-person meeting with him. If your sessions are on-line, then scan all your completed pages together into ONE pdf file for each person (preferably not separate files).
