Be sure to complete and sign all of the forms including both of you signing the HIPAA form on the back page. Pay close attention that BOTH of you fill in or sign all of the starred blanks on the “No Surprise Billing form. Some of the required information on that form will be redundant but we are following strict federal regulations.
Scan all your completed pages together into ONE PDF file, not separate files. Adobe Scan can do this easily. After you have created your pdf file, then come back to this same page and attach your file to the bottom of the following encrypted file transfer mechanism.
